Let's start your journey, together

Navigating the ongoing support journey for another person can feel confusing, but you shouldn’t have to figure it out alone. Symbol makes the process clearer, calmer, and easier to manage. We guide you through each step, helping you understand what options are available so you can ensure your loved one receives premium support that is tailored to their specific wants and needs.

This page shines a light on:

  • Where to start if you’re unsure what kind of support they desire
  • How to register someone with My Aged Care (with or without their presence)
  • What information you’ll need to act on their behalf
  • How Symbol can support you through assessments, approvals and choosing services
  • Common scenarios for people looking on behalf of someone else
  • How to navigate tricky conversations with a person you care about

Before anything formal begins, it helps to observe and understand what’s happening in their daily life

What you can ask:

  • Are they finding certain tasks more effort than usual?
  • Are they feeling lonely or isolated?
  • Are they recovering from an injury or hospital stay?
  • Do they have any concerns?

To receive government-funded support, the person needs to be registered with My Aged Care. You could register with them or on their behalf – as long as you have their permission.

If you need help with registration, our onboarding specialist can walk you through the process step by step.

What My Aged Care will ask for:

  • Their full name and date of birth
  • Medicare details
  • Address and contact details
  • Your relationship to them

If it’s needed, you may be formally listed as their representative by applying for Enduring Power of Guardianship. Symbol can help you complete this setup correctly so nothing is missed.

This will allow you to:

  • Speak to My Aged Care on behalf of them
  • Help manage their assessments
  • Choose their aged care provider
  • Coordinate their supports

Before getting funding, you will be visited by an ACAT assessor. It is important to be prepared so you can receive the right level of funding required. Symbol can help you prepare, so the conversation is clear, calm and aligned with what they really want.

An ACAT assessor will talk through:

  • How the person is managing daily tasks
  • What’s becoming difficult
  • What support would help
  • Their goals and preferences

When their funding becomes available, you’ll have 56–84 days to choose a provider.

Symbol offers:

  • Clear, human explanations
  • A dedicated onboarding specialist
  • Support in building a personalised care plan
  • Help balancing their wants, needs and lifestyle
  • A companion-style approach to care, not a clinical one

Common scenarios we hear

Mum’s not coping at home but doesn’t want help.

We help families have the hard conversation in a way that feels supportive, not like she's losing her independence.

Dad was discharged from hospital and we don’t know what happens next.

We can step in quickly, stabilise the situation, and walk you through your short-term care options.

My partner has memory changes and I feel overwhelmed.

Symbol supports you as the carer too, alongside dementia-aware care that's calm, patient and genuinely compassionate.

I live far away — I need someone reliable and warm.

You'll have the same familiar face each visit, so there's continuity, real rapport, and regular updates for you.

I don't know where to start with My Aged Care.

We'll walk you through the assessment and funding process step by step, so you're never figuring it out alone.

We're worried about Mum falling or feeling unsafe.

We can arrange a home safety check and more frequent visits, so small risks get caught before they become big ones.
